We are a 501(c)3 non-profit charitable organization that is able to provide pharmacist-embedded team based care. We provide necessary community services and public health campaigns for all individuals, regardless of where they were born, race, gender, ethnicity, age, or socioeconomic class.
Our belief is that all people should have equal access to healthcare. By identifying areas of inequity and providing them with quality care, we will be able to ameliorate healthcare outcomes and eliminate the gap in our current healthcare system.

Our goal is to unite pharmacists, physicians and patients and strengthen the collaboration. Pharmacist-physician-patient relationship can improve adherence to medication, significantly increase patient outcomes and enhance physician office quality measures. It is very important to empower pharmacists to allow them to be an essential part of the healthcare team.
We support healthcare teams, payers, and government entities in their efforts to improve preventative care and patient outcomes.
